目的 通过观察氦溴索对机械通气大鼠肺组织肺表面活性蛋白A(SP-A)和髓过氧化物酶(MPO)表达水平的影响,探讨氨溴索对呼吸机所致肺损伤(VILI)的防治作用.方法 Wistar大鼠24只,随机分为对照组、机械通气组(MV组)和氨溴索干预组(AMB组).观察各组大鼠肺组织病理学改变,测定支气管肺泡灌洗液(BALF)中性粒细胞计数,采用考马斯亮蓝法测定BALF蛋白含量,采用MPO检测试剂盒测定BALF和肺组织MPO活性,采用反转录聚合酶链反应和免疫组织化学染色方法测定肺组织SP-A mRNA及其蛋白表达.结果 MV组大鼠BALF中性粒细胞计数、BALF和肺组织MPO活性较对照组和AMB组明显升高(P值均<0.01),而肺组织SP-A mRNA及其蛋白表达水平较对照组和AMB组明显降低(P值均<0.01);对照组上述指标与AMB组比较差异无统计学意义.相关性分析显示,肺组织SP-A水平与MPO活性呈负相关(r=-0.775,P<0.05).结论 SP-A和MPO在VILI的发生发展中具有重要作用,氨溴索可通过降低机械通气大鼠肺组织MPO活性,促进SP-A表达,对VILI有一定保护作用.
