目的观察人血白蛋白与纳络酮联合治疗急性大面积脑梗死的疗效及安全性。方法2007年7月至2009年5月本院36例急性大面积脑梗死患者,随机分为治疗组和对照组各18例。治疗组应用人血白蛋白与纳络酮联合治疗;对照组应用常规脱水降颅压及活血化瘀、抗血小板聚集和奥拉西坦治疗。评价两组治疗前、治疗后7,14,21,28d的神经功能缺损评分和日常生活能力评分。结果治疗后14d治疗组神经功能缺损评分和日常生活能力改善均较对照组明显(P<0.05),有效率高于对照组,死亡率明显降低;治疗后28d,治疗组和对照组神经功能缺损评分的有效率分别为44.4%和27.7%(P<0.01),日常生活能力评分的有效率分别为61%和27.7%(P<0.01)。不增加不良反应。结论人血白蛋白与纳络酮联合治疗能有效改善急性大面积脑梗死患者神经功能缺损和日常生活能力,未见明显不良反应。
Objective To observe the efficacy and safety of human serum albumin and naloxone in the treatment of acute large area cerebral infarction. Methods From July 2007 to May 2009, 36 patients with acute large area cerebral infarction in our hospital were randomly divided into treatment group (n = 18) and control group (n = 18). Treatment group, the application of human serum albumin and naloxone combined treatment; control group using conventional dehydration and intracranial pressure and blood circulation, anti-platelet aggregation and oxiracetam treatment. The scores of neurological deficit score and daily living ability of the two groups before treatment, 7,14,21,28d after treatment were evaluated. Results After 14 days of treatment, the neurological deficit score and daily living ability of the treatment group were significantly improved (P <0.05), and the effective rate was significantly lower than that of the control group. At 28 days after treatment, the neurological deficits in the treatment group and the control group The effective rates were 44.4% and 27.7% respectively (P <0.01), and the scores of daily living ability were 61% and 27.7% respectively (P <0.01). Do not increase the adverse reaction. Conclusion Combined treatment of human serum albumin and naloxone can effectively improve the neurological deficits and daily living ability of patients with acute large area cerebral infarction without obvious adverse reactions.
