以百白破疫苗为例探讨北京市朝阳区疑似预防接种异常反应主动监测模式

目的分析无细胞百白破疫苗(DTa P)接种后疑似预防接种异常反应(adverse event following immunization,AEFI)主动监测和被动监测结果,评价主动监测模式的敏感性。方法通过全国AEFI信息管理系统,收集北京市朝阳区2010~2013年4~6月DTa P AEFI被动监测个案数据;2013年4~6月对朝阳区4家试点社区接种DTa P的儿童实施主动监测,收集AEFI个案数据;比较试点社区AEFI主动监测与同期自身被动监测、同期对照社区被动监测、自身历年被动监测、历年朝阳区被动监测的情况。结果主动监测AEFI报告率为8 561.24/10万,高于同期自身被动监测、同期对照社区被动监测、自身历年被动监测、历年朝阳区被动监测报告率,差异均有统计学意义(P均<0.001);主动监测一般反应报告率为7 491.08/10万,异常反应报告率为118.91/10万,偶合症报告率为951.25/10万,均高于被动监测,差异均有统计学意义(P均<0.01)。结论 AEFI主动监测敏感性好,与被动监测相比,更能客观、真实、完整地反映疫苗接种后AEFI信息。 Objective To analyze the results of active and passive monitoring of suspected AEFI after inoculated with DTa P and evaluate the sensitivity of active monitoring. Methods The data of passive monitoring of DTa P AEFI from April to June in 2010 in Chaoyang District of Beijing were collected through the national AEFI information management system. From April to June 2013, active monitoring was performed on children vaccinated with DTa P in 4 pilot communities in Chaoyang District The data of AEFI were collected and compared with the passive monitoring of AEFI in the pilot communities and passive monitoring in the same period. Passive monitoring of the AEFI in the pilot communities, passive monitoring in the past years and passive monitoring in the Chaoyang District were conducted in the past years. Results The reported rate of active monitoring of AEFI was 8 561.24 / 100 000, which was higher than that of passive monitoring during the same period, passive monitoring of control community over the same period, passive monitoring over its own calendar year, and passive monitoring reporting rate of Chaoyang District in the past years ). The average response rate of active surveillance was 7 491.08 / lakh, the rate of abnormal response was 118.91 / lakh and the incidence of intercourse was 951.25 / lakh, both of which were higher than those of passive monitoring <0.01). Conclusion AEFI has good sensitivity in active surveillance. Compared with passive monitoring, AEFI can more objectively, accurately and completely reflect AEFI information after vaccination.
