七氟醚吸入麻醉在非住院小儿眼科手术中的应用

目的:观察七氟醚吸入麻醉在非住院小儿眼科手术中的应用效果。方法:选择ASAⅠ~Ⅱ级、年龄6~24月因新生儿泪囊炎、霰粒肿需行手术治疗的患儿60例,随机分为两组:七氟醚组(S组)及氯胺酮组(K组),分别采用面罩吸入七氟醚或肌肉注射氯胺酮进行麻醉诱导及维持。观察记录两组患者麻醉中平均动脉压(MAP)、心率(HR)、呼吸(R)及脉搏血氧饱和度(SpO2)的变化,记录意识消失、苏醒及离开麻醉恢复室的时间,观察并随访麻醉中及麻醉后的不良反应。结果:S组患者术中MAP轻度下降,HR变化不明显,而K组术中MAP及HR均明显升高;S组意识消失时间、苏醒时间及离开麻醉恢复室的时间明显短于K组;K组诱导期兴奋、术中体动及分泌物增加的病例数明显多于S组。结论:七氟醚吸入麻醉具有麻醉诱导迅速、维持平稳、苏醒彻底的特点,可安全用于非住院小儿眼科手术的麻醉。 Objective: To observe the effect of sevoflurane inhalation anesthesia in non-hospital pediatric ophthalmic surgery. Methods: Sixty children with ASA Ⅰ ~ Ⅱ grade and 6 ~ 24 months old due to neonatal dacryocystitis and chalazion surgery were randomly divided into two groups: sevoflurane group (S group) and ketamine group Group K). Respiratory induction of sevoflurane and intramuscular injection of ketamine were performed respectively by mask inhalation and maintenance. The change of mean arterial pressure (MAP), heart rate (HR), respiration (R) and pulse oximetry (SpO2) were observed and recorded during the anesthesia in the two groups. The time of consciousness disappearing, waking up and leaving the anesthesia recovery room was recorded. Follow-up anesthesia and anesthesia after adverse reactions. Results: The intraoperative MAP of S patients decreased slightly and HR did not change obviously, while the MAP and HR of K patients were significantly increased in S group. The disappearance time, recovery time and leaving anesthesia recovery room in S group were significantly shorter than those in K group ; K group excitement during the excitement, surgery and secretions increase the number of cases was significantly more than the S group. Conclusion: Sevoflurane inhalation anesthesia has the characteristics of rapid anesthesia induction, stable and complete recovery, and can be safely used for anesthesia in non-hospital pediatric ophthalmic surgery.
