目的探讨复方丹参联合硫酸镁治疗妊娠合并慢性高血压并发先兆子痫的临床疗效。方法选取2013年1月—2015年6月禹州市妇幼保健院收治的妊娠合并慢性高血压产妇100例,随机分为对照组与观察组,各50例。对照组予以硝苯地平+硫酸镁治疗,观察组从妊娠第4个月予以硝苯地平+复方丹参+硫酸镁治疗,观察两组产妇先兆子痫的发生情况、并发症发生情况以及围生儿情况。并发症包括产后出血、剖宫产、视网膜病变、胎盘剥离和肺水肿;围生儿情况包括新生儿死亡、早产、死胎、新生儿窒息和小于胎龄儿发生情况。结果观察组产妇先兆子痫的发病率和早发型先兆子痫的发病率低于对照组,差异有统计学意义(P<0.05);观察组产妇视网膜病变、胎盘剥离和肺水肿的发生率低于对照组,差异有统计学意义(P<0.05);两组产妇产后出血、剖宫产发生率比较,差异无统计学意义(P>0.05);观察组中新生儿死亡、早产、死胎和新生儿窒息的发生率低于对照组,差异有统计学意义(P<0.05);两组产妇中小于胎龄儿的发生率比较,差异无统计学意义(P>0.05)。结论复方丹参联合硫酸镁治疗妊娠合并慢性高血压并发先兆子痫的临床疗效显著,能有效预防先兆子痫的发生,降低胎盘剥离等发生率,且不会增加产后出血及小于胎龄儿的发生率。
Objective To investigate the clinical efficacy of compound Danshen combined with magnesium sulfate in the treatment of pregnancy complicated with chronic hypertension complicated with preeclampsia. Methods From January 2013 to June 2015, 100 maternal pregnant women complicated with chronic hypertension admitted to Yuzhou Maternal and Child Health Hospital were randomly divided into control group and observation group, with 50 cases each. The control group was treated with nifedipine + magnesium sulfate. The observation group was treated with nifedipine + compound salvia miltiorrhiza + magnesium sulfate in the fourth month of gestation. The incidence of preeclampsia, complications and perinatal morbidity Happening. Complications included postpartum hemorrhage, cesarean section, retinopathy, placental exfoliation, and pulmonary edema. Perinatal conditions included neonatal death, premature birth, stillbirth, neonatal asphyxia, and small gestational age. Results The incidence of preeclampsia and premature eclampsia in observation group were lower than those in control group (P <0.05). The incidences of retinopathy, placental exfoliation and pulmonary edema in observation group were lower (P <0.05). There was no significant difference in the incidence of postpartum hemorrhage and cesarean section between the two groups (P> 0.05). The neonatal mortality, premature birth and stillbirth in the observation group were significantly higher than those in the control group The incidence of neonatal asphyxia was lower than that of the control group, the difference was statistically significant (P <0.05). There was no significant difference in the incidence of neonatal asphyxia between the two groups (P> 0.05). Conclusion Compound Danshen combined with magnesium sulfate in pregnancy complicated with chronic hypertension complicated by preeclampsia has a significant clinical effect, which can effectively prevent the occurrence of preeclampsia and reduce the incidence of placental exfoliation, and will not increase the incidence of postpartum hemorrhage and small gestational age children rate.
