目的探讨维持性血液透析患者血磷水平及其相关临床因素。方法选择2014年6—12月于本院门诊行维持性血液透析的95例患者同一时期透析后检测的临床指标,分析血磷水平及与其它临床指标的关系。计量资料比较采用t检验,两指标相关性分析采用Pearson相关,P<0.05为差异有统计学意义。结果两组患者年龄[(53.20±12.16)、(60.68±12.17)岁]、血浆白蛋白[(36.75±5.44)、(77.64±3.48)g/L]、血红蛋白[(102.31±20.01)、(111.93±21.69)g/L]、红细胞压积[(0.311±0.062)、(0.344±0.069)]、铁蛋白[(379.49±42.54)、(543.89±44.78)μ/L]、甲状旁腺素[(301.01±46.67)、(603.11±80.81)pg/ml]、血肌酐[(716.71±317.79)、(1 048.11±287.30)μmol/L]比较,差异均有统计学意义(均P<0.05)。血磷与年龄、血红蛋白、红细胞压积、血清铁、铁蛋白、甲状旁腺素、血肌酐均呈正相关(r=0.363、0.208、0.227、0.338、0.250、0.265、0.604,均P<0.05)。结论在维持性血液透析患者中高磷血症的发生率高,且与患者营养吸收、透析充分性及继发性甲状旁腺功能亢进等因素相关。
Objective To investigate the level of serum phosphorus and its related clinical factors in maintenance hemodialysis patients. Methods Clinical data of 95 patients with maintenance hemodialysis in our hospital from June to December in 2014 were analyzed after dialysis, and the relationship between serum phosphorus level and other clinical indexes was analyzed. Measurement data were compared using t test, correlation analysis of two indicators using Pearson correlation, P <0.05 for the difference was statistically significant. Results The mean age of the patients in the two groups was 53.20 ± 12.16 and 60.68 ± 12.17 years respectively, and the plasma levels of albumin, albumin and albumin were (36.75 ± 5.44 and 77.64 ± 3.48) g / L and [102.31 ± 20.01 and 111.93 ± 21.69 g / L], hematocrit [(0.311 ± 0.062), (0.344 ± 0.069)], ferritin [(379.49 ± 42.54), (543.89 ± 44.78) μL], parathyroid hormone [ 301.01 ± 46.67), (603.11 ± 80.81) pg / ml] and serum creatinine [(716.71 ± 317.79), (1 048.11 ± 287.30) μmol / L], all of which were statistically significant (all P <0.05). Serum phosphorus and age, hemoglobin, hematocrit, serum iron, ferritin, parathyroid hormone, serum creatinine were positively correlated (r = 0.363,0.208,0.227,0.338,0.250,0.265,0.604, all P <0.05). Conclusions The incidence of hyperphosphatemia in maintenance hemodialysis patients is high, and is related to such factors as nutritional intake, adequacy of dialysis and secondary hyperparathyroidism.
