目的探讨HPV L1蛋白在宫颈脱落细胞病变中的表达情况及其临床意义。方法以非同位素标记核酸分子杂交技术和免疫组织/细胞化学技术为基础,在同时采用核酸和免疫水平双重检测传染病病原体人乳头瘤病毒壳蛋白抗体(HPV L1)的方法对124例妇科宫颈脱落细胞TCT标本进行HPV L1蛋白检测。结果 HPV L1蛋白的总阳性率为44.35%(55/124)。其中轻度炎症的阳性率为66.67%(26/39);中度炎症的阳性率为41.94%(13/31);重度炎症的阳性率为37.50%(6/16);低级别上皮内瘤变的阳性率为30.43%(7/23);高级别上皮内瘤变的阳性率为20.00%(3/15)。HPV L1蛋白在轻度炎症的阳性表达率高于中度炎症和重度炎症,明显高于低级别上皮内瘤变和高级别上皮内瘤变,差异具有统计学意义(χ2=13.65,P<0.05)。结论 HPV L1蛋白的阳性表达与宫颈脱落细胞病变的类型密切相关,随着宫颈脱落细胞病变的加深,HPV L1蛋白的阳性率逐渐减弱,HPV L1蛋白的丢失可能是宫颈病变进展的一个早期事件,HPV L1蛋白的阳性表达与宫颈病变的生物学行为密接相关,可作为宫颈病变治疗预后的可靠理论依据。
Objective To investigate the expression of HPV L1 protein in cervical exfoliative cytopathology and its clinical significance. Methods Non-isotope-labeled nucleic acid hybridization and immunohistochemistry were used to detect HPV L1 in nude mice by double detection of nucleic acid and immunological level. Cell TCT specimens for HPV L1 protein detection. Results The total positive rate of HPV L1 protein was 44.35% (55/124). The positive rate of mild inflammation was 66.67% (26/39); the positive rate of moderate inflammation was 41.94% (13/31); the rate of severe inflammation was 37.50% (6/16); the low grade intraepithelial neoplasia The positive rate was 30.43% (7/23). The positive rate of high grade intraepithelial neoplasia was 20.00% (3/15). The positive rate of HPV L1 protein in mild inflammation was higher than that of moderate inflammation and severe inflammation, which was significantly higher than that of low grade intraepithelial neoplasia and high grade intraepithelial neoplasia (χ2 = 13.65, P <0.05) ). Conclusion The positive expression of HPV L1 protein is closely related to the type of cervical exfoliated cells. With the exacerbation of cervical exfoliated cells, the positive rate of HPV L1 protein gradually decreases. The loss of HPV L1 protein may be an early event of cervical lesion progression. The positive expression of HPV L1 protein is closely related to the biological behavior of cervical lesions and can be used as a reliable theoretical basis for the prognosis of cervical lesions.
