目的:研究不同程度宫颈病变者人乳头瘤病毒(HPV)的感染率及HPV的感染亚型,探讨不同基因亚型的HPV感染与子宫颈病变的关系。方法:2008年6月~2009年6月杭州市第一人民医院妇科门诊行宫颈活检的519例患者,采用核酸分子快速杂交基因分型技术进行21种HPV-DNA亚型分析。宫颈活检患者根据病理结果分为炎症组、低度鳞状上皮内瘤变组、高度鳞状上皮内瘤变组、宫颈浸润癌组。结果:519例患者有HPV感染者254例,其中96例为多重HPV感染。HPV亚型感染频度由高到低前五位分别为16、58、33、18、52,高危型HPV亚型的检出率随宫颈病变严重程度的增加而上升,HPV的二重及二重以上感染在高度鳞状上皮内瘤变组及宫颈浸润癌组明显高于炎症组和低度鳞状上皮内瘤变组,差异有统计学意义(P<0.01)。结论:HPV感染尤其是高危型HPV亚型感染与宫颈癌关系密切,多重HPV感染者所患宫颈病变更趋于严重。
Objective: To investigate the infection rate of human papillomavirus (HPV) and HPV subtypes in patients with cervical lesions of different degrees and to explore the relationship between HPV infection and cervical lesions of different gene subtypes. Methods: From June 2008 to June 2009, 519 patients underwent cervical biopsy in the Gynecology Clinic of Hangzhou First People’s Hospital. 21 HPV-DNA subtypes were analyzed by rapid hybridization genotyping using nucleic acid. Cervical biopsies were divided into inflammatory group, low-grade squamous intraepithelial neoplasia group, highly squamous intraepithelial neoplasia group and cervical invasive carcinoma group according to the pathological results. Results: There were 254 cases of HPV infection in 519 cases, of which 96 cases were multiple HPV infection. The frequency of HPV subtype infection from high to low top five were 16,58,33,18,52 respectively. The detection rate of high-risk HPV subtypes increased with the severity of cervical lesions, The above infection was significantly higher in severe squamous intraepithelial neoplasia group and invasive cervical cancer group than in inflammatory group and low grade squamous intraepithelial neoplasia group (P <0.01). Conclusion: HPV infection, especially high-risk HPV subtype infection, is closely related to cervical cancer. Cervical lesions in patients with multiple HPV infections tend to be more serious.
