目的探讨胃镜下大蒜素局部应用对进展期胃癌的影响及作用机制。方法进展期胃癌患者80例,胃镜及病理均证实为腺癌,分为两组;大蒜素组40例,术前48h胃镜下病变处局部注射大蒜素,对照组40例局部注射生理盐水;手术切除胃癌组织,采用美国Becton Dickinson公司产的FACS-420型流式细胞仪检测,观察胃癌组织周期(G0/G1期、S期、G2/M期)各时相百分比、细胞凋亡率、增殖指数(proliferation in-dex,PI),以及胃癌组织细胞凋亡相关基因(Fas、Bax、Bcl-2)蛋白表达。结果大蒜素组与对照组细胞凋亡率(%)分别为9.60±1.52、2.20±0.58,G0/G1期(%)分别为72.12±8.35、69.56±5.15,G2/M期(%)分别为9.54±3.20、13.20±3.05,PI值分别为27.80±8.35、30.40±5.15;两组比较,差异均有统计学意义(P<0.05,P<0.01);大蒜素组可增加凋亡促进基因Bax基因蛋白、凋亡始动基因Fas基因蛋白的表达(P<0.05,P<0.01),降低凋亡抑制基因Bcl-2基因蛋白的表达(P<0.05)。结论本项研究初步证明胃镜下局部注射大蒜素能够抑制进展期胃癌组织细胞生长、增殖,并促进胃癌细胞凋亡。
Objective To investigate the effect of topical application of allicin under endoscopy on advanced gastric cancer and its mechanism. Conclusion This study preliminarily proved that local injection of allicin under endoscopy can inhibit the growth and proliferation of advanced gastric cancer cells and promote the apoptosis of gastric cancer cells.
