目的研究粘着斑激酶 (FAK)表达与大肠癌发生、生物学行为和预后的关系。方法应用免疫组织化学S -P方法检测 10例正常肠粘膜、16例肠腺瘤伴不典型增生及 10 8例大肠癌手术标本中FAK的表达。结果正常肠粘膜表达均阴性、肠腺瘤伴不典型增生表达阳性率为 12 .5 0 % ,大肠癌表达阳性率为 75 .93% (82 / 10 8)。其中无区域淋巴结转移者阳性率为 48.84% ,有区域淋巴结转移者阳性率为 93.85 % (P <0 .0 1) ,FAK表达水平与大肠癌浸润深度和Dukes’分期呈正相关 (均 P<0 .0 1)。FAK阳性者术后 3年、5年生存率分别为 49.74%、44 .30 %。而阴性者为 10 0 %、88.5 4%。二组患者术后 3年、5年生存率差别均有显著性意义 (均P <0 .0 1)。结论大肠癌由于FAK表达增加 ,加速恶性细胞的增殖、浸润和转移。故检测大肠活检粘膜及癌组织中FAK表达状况 ,有助于进一步了解大肠粘膜癌变倾向及大肠癌生物学行为 ,有利于对患者预后的判断。
Objective To investigate the relationship between the expression of focal adhesion kinase (FAK) and the occurrence, biological behavior and prognosis of colorectal cancer. Methods Immunohistochemical S-P method was used to detect the expression of FAK in 10 cases of normal intestinal mucosa, 16 cases of intestinal adenoma with atypical hyperplasia and 108 cases of colorectal cancer. Results The expression of normal intestinal mucosa was negative, the positive rate of intestinal adenoma with atypical hyperplasia was 12.5%, and the positive rate of colorectal carcinoma was 75.93% (82/108). The positive rate of regional lymph node metastasis was 48.84%, the positive rate of regional lymph node metastasis was 93.85% (P <0.01). The expression of FAK was positively correlated with the depth of invasion and Dukes’ stage (all P <0) .0 1). FAK positive 3 years after surgery, 5-year survival rates were 49.74%, 44 .30%. While the negative ones were 10 0% and 88.5 4%. The difference of 3-year and 5-year survival rate between the two groups was significant (P <0.01). Conclusions Colorectal cancer accelerates the proliferation, invasion and metastasis of malignant cells due to the increase of FAK expression. Therefore, detection of colorectal mucosa and cancer tissue FAK expression status, help to further understand the tendency of colorectal cancer and colorectal cancer biological behavior, is conducive to the prognosis of patients.
