目的了解江苏省病毒性肝炎患者住院费用,探究影响因素,为合理控制其住院费用、切实缓解患者的经济负担提供参考。方法收集江苏省某三级乙等医院2013年10月—2016年4月因病毒性肝炎及其并发症收治住院患者的住院信息,对病例的住院费用特点及其影响因素进行统计分析。结果共收集病毒性肝炎患者778例,男性561例(72.11%),女性217例(27.89%),平均年龄为(41.79±19.02)周岁;病毒性肝炎患者在病原学的5个分型上以乙型肝炎(70.23%)和戊型肝炎(21.84%)为主;甲型肝炎的住院天数最短(5.5 d),戊型肝炎的住院天数最长(16 d);次均住院费用为7 063.47元,其中戊型肝炎费用最高(10 582.78元),其次为丙型肝炎(9 272.37元)、乙型肝炎(7 829.61元),费用最低的为甲型肝炎(2 294.19元);分析结果显示,性别、年龄、支付方式、病原学分型、是否合并其他诊断、住院天数及出院情况是住院费用的影响因素。结论合理缩短住院天数,进一步加大针对老年群体的社会基本医疗保险投入,有利于缓解病毒性肝炎患者的经济负担。
Objective To understand the cost of hospitalization for patients with viral hepatitis in Jiangsu Province and explore the influencing factors so as to provide reference for the reasonable control of their hospitalization expenses and practical relief of the patients’ financial burden. Methods The hospitalization information of hospitalized patients from a tertiary level B hospital in Jiangsu province from October 2013 to April 2016 was collected from patients with viral hepatitis and its complications. The hospitalization expenses characteristics and influencing factors were analyzed statistically. Results A total of 778 cases of viral hepatitis were collected, 561 cases (72.11%) were male and 217 cases (27.89%) were female, mean age was (41.79 ± 19.02) years old. Among the 5 types of viral hepatitis Hepatitis B (70.23%) and hepatitis E (21.84%), the shortest duration of hospitalization for hepatitis A (5.5 days) and the longest period of hospitalization for hepatitis E (16 days), and the average hospitalization expense was 7 063.47 The highest cost of hepatitis E (10 582.78 yuan) was followed by hepatitis C (9 272.37 yuan), hepatitis B (7 829.61 yuan) and the lowest cost hepatitis A (2 294.19 yuan) Sex, age, payment method, etiological classification, whether or not to combine other diagnosis, hospitalization days and hospital discharge were the influencing factors of hospitalization expenses. Conclusions Reasonably shorten the length of stay and further increase the investment of social basic medical insurance for the elderly, which will help alleviate the economic burden of patients with viral hepatitis.
