目的:探讨丙型肝炎患者血清HCVRNA病毒载量与肝脏功能生化指标变化的关系。方法:对180例慢性丙型肝炎患者和30例健康体检者进行分组,其中A组30例为健康体检者,B组60例为抗-HCV阳性,HCVRNA含量<1×103拷贝/毫升,C组80例为抗-HCV阳性,HCVRNA含量在1×103~9.99×105拷贝/毫升,D组40例为抗-HCV阳性,HCVRNA含量在≥1×106拷贝/毫升。对210例标本同时进行抗-HCV、HCV-RNA定量、肝功能(ALT、AST)检测,分别比较各组间关系。结果:A组30例体检者ALT正常27例异常3例,AST均正常。B组60例ALT正常51例异常9例,AST正常55例异常5例。C组80例ALT正常20例异常60例,AST正常27例异常53例。D组40例ALT正常3例异常37例,AST正常5例异常35例。A组与B组之间肝功能比较差别无统计学意义(P>0.05),A组、B组分别与C组、D组之间肝功能比较差别有统计学意义(P<0.05),C组与D组之间肝功能比较差别有统计学意义(P<0.05)。结论:慢性丙型肝炎血清HCVRNA病毒复制水平与肝脏功能ALT、AST异常表达相关,慢性丙型肝炎肝功能异常最直接因素可能为HCVRNA的复制所导致。
Objective: To investigate the relationship between serum hepatitis C virus (HCV) viral load and changes of liver biochemical markers in patients with hepatitis C Methods: 180 patients with chronic hepatitis C and 30 healthy subjects were divided into groups, of which 30 cases were healthy in group A, 60 cases in group B were anti-HCV positive, HCVRNA content <1 × 103 copies / ml, C Group 80 were positive for anti-HCV, HCV RNA content of 1 × 103 ~ 9.99 × 105 copies / ml, D group of 40 cases were anti-HCV positive, HCVRNA content of ≥ 1 × 106 copies / ml. The anti-HCV, HCV-RNA quantitation and liver function (ALT, AST) were detected in 210 cases at the same time, and the relationships among groups were compared. Results: A group of 30 patients with normal ALT 27 cases were abnormal in 3 cases, AST were normal. Group B 60 cases of normal ALT in 51 cases of abnormal 9 cases, normal AST 55 cases of abnormal 5 cases. 80 cases in group C had 20 cases with normal ALT, 60 cases with abnormal, and 27 cases with normal AST in 53 cases. D group, 40 cases of ALT normal in 3 cases of abnormal 37 cases, AST normal 5 cases of abnormal 35 cases. There was no significant difference in liver function between group A and group B (P> 0.05), and the difference of liver function between group A and group B and group C and group D was statistically significant (P <0.05), C The difference of liver function between group and group D was statistically significant (P <0.05). Conclusion: The level of HCV RNA replication in patients with chronic hepatitis C is related to the abnormal expression of liver function ALT and AST. The most direct factor of hepatic dysfunction in chronic hepatitis C may be the replication of HCV RNA.
