产妇26岁,以足月妊娠,胎儿娩出,胎盘不下9h 急诊入院。9h 前自然分娩1胎,无人接生,胎儿头位娩出,哭声好,体重约3000g,产后阴道出血约500ml,1、2产程约2h。胎儿娩出后1h 胎盘不下,当地医生助娩失败后转来。入院时产妇自觉口渴、头昏、排尿困难。查体:T36℃、BP12/8kPa,P130次/min,R24次/min,神志清,精神差,贫血貌,心肺无异常,腹平软,宫高脐下2指偏右,膀胱充盈。妇科检查:外阴血染,脐带外露约12cm,阴道畅、完整、积血块
