目的 评价食管癌原发灶SUVmax、SUVmeanMTV、TLG与临床分期的相关性,初步探讨转移淋巴结SUVmax与原发灶SUVmax、SUVmean、MTV、TLG及临床分期的关系.方法 回顾性分析161例(男130例,女31例,年龄41~ 90岁)已确诊的食管癌患者资料.所有患者均于治疗前行18F-FDGPET/CT检查,测定食管癌原发灶的SUVmax、SUVmean、MTV,并计算TLG.对于有淋巴结转移的患者,选取FDG摄取最高的转移淋巴结,测定其SUVmax.采用单因素方差分析及两独立样本t检验进行组间比较.采用Spearman相关分析评价各参数与TNM分期及临床分期的相关性.结果 (1)161例食管癌原发灶的SUVmax、SUVmean、MTV、TLG分别为9.9±4.0、6.2±2.6、(12.5±11.2) cm3、(85.3±84.7)g.转移淋巴结的SUVmax为6.0±3.5.(2)不同病理类型、T分期、N分期的食管癌原发灶的SUVmax差异均有统计学意义(F=5.030和10.281,t=4.169,均P<0.05),有淋巴结转移(N1期)患者高于无淋巴结转移(N0期)患者,鳞状细胞癌高于腺癌.(3) SUVmax、SUVmean、MTV、TLG均与T分期呈正相关(rs=0.290、0.285、0.446和0.481,均P<0.05).SUVmax、SUVmean、TLG均与N分期呈正相关(rs=0.237、0.230和0.204,均P<0.05).各参数与M分期均不存在相关性(rs=0.029~0.074,均P>0.05).(4)食管癌转移淋巴结的SUVmax与原发灶的SUVmax、SUVmean、TLG均呈正相关(rs=0.269、0.249和0.232,均P<0.05),而与MTV不存在相关性(rs=0.170,P>0.05).转移淋巴结的SUVmax与T分期不存在相关性(rs=0.060,P>0.05),但与M分期及临床分期呈正相关(rs=0.280和0.298,均P<0.05).结论 食管癌原发灶的SUVmax、SUVmean、TLG、MTV均与T分期呈正相关,其中前3项参数还与N分期呈正相关.食管癌转移淋巴结的SUVmax与原发灶的SUVmax、SUVmean、TLG、M分期及临床分期均呈正相关.“,”Objective To investigate the correlations between metabolic indexes (SUVmax,SUVmean,MTV,TLG) of esophageal cancer primary lesion and clinical staging,and those between SUVmaxof metastatic lymph node and metabolic indexes of primary lesion,clinical staging.Methods A total of 161 newly biopsy-proven esophageal carcinoma patients (130 males,31 females,age:41-90 years) were enrolled into this retrospective study.All underwent 18F-FDG PET/CT before treatment.SUVmax,SUVmean and MTV of primary lesion were measured,TLG was calculated and the highest SUVmax of metastatic lymph nodes was measured.Data were compared using one-way analysis of variance and two-sample t test.Spearman correlation analysis was used to evaluate correlations between metabolic indexes and clinical staging.Results (1) SUVmax,SUVmean,MTV and TLG of 161 primary lesions were 9.9±4.0,6.2±2.6,(12.5±11.2) cm3 and (85.3±84.7) g respectively.SUVmax of metastatic lymph nodes was 6.0±3.5.(2) There were statistically significant differences in SUVmax of different pathological types,T stages,N stages (F=5.030,10.281,t =4.169; all P<0.05).SUVmax of squamous cell carcinoma was higher than that of adenocarcinoma.SUVmax of N1 patients was higher than that of N0 patients.(3) SUVmax,SUV MTV and TLG were all positively correlated with T stage (rs =0.290,0.285,0.446,0.481 ; all P<0.05).SUVmax,SUVmean and TLG were all positively correlated with N stage (rs =0.237,0.230,0.204; all P<0.05).There was no correlation between each parameter and M stage (rs =0.029-0.074; all P>0.05).(4) SUVmax of metastatic lymph nodes was positively correlated with SUVmax,SUV TLG of primary lesion (rs =0.269,0.249,0.232; all P <0.05),but not with MTV (rs =0.170,P>0.05).SUVmax of metastatic lymph nodes was positively correlated with M stage and clinical stage (rs =0.280,0.298; both P<0.05),but not with T stage(rs =0.060,P>0.05).Conclusions SUVmax,SUVmean,MTV and TLG of esophageal cancer primary lesion were all positively correlated with T stage.SUVmax,SUVmean and TLG were also positively correlated with N stage.SUVmax of metastatic lymph nodes was positively correlated with SUVmax,SUV TLG of primary lesion,M stage and clinical staging.
