目的探讨血清白蛋白浓度在小儿急性腹泻病中的临床意义。方法 243例小儿急性腹泻病患儿,根据是否合并轻度胃肠炎伴良性婴幼儿惊厥(BICE),是否合并脓毒症,不同简易小儿危重症评分法(PCIS)评分(>72分或≤72分)及不同住院时间(≥7 d或<7 d)对患儿进行分组,比较不同组别入院24 h内血清白蛋白浓度差异。结果 (1)本组患儿合并脓毒症为66例(27.2%),平均血清白蛋白浓度为(37.430±4.925)g/L;未合并脓毒症177例(72.8%),平均血清白蛋白浓度为(40.690±4.338)g/L,两组血清白蛋白浓度比较差异具有统计学意义(t=5.019,P<0.05)。(2)本组患儿合并BICE为29例(11.9%),平均血清白蛋白浓度为(40.090±3.504)g/L;未合并BICE 214例(88.1%),平均血清白蛋白浓度为(39.760±4.871)g/L,两组血清白蛋白浓度比较差异无统计学意义(t=0.352,P>0.05)。(3)本组患儿中,PCIS评分≤72分21例(8.6%),平均血清白蛋白浓度为(36.670±4.643)g/L;PCIS评分>72分222例(91.4%),平均血清白蛋白浓度为(40.100±4.632)g/L,两组血清白蛋白浓度比较差异具有统计学意义(t=3.243,P<0.05)。(4)243例患儿中住院时间≥7 d 46例(18.9%),平均血清白蛋白浓度(38.130±4.135)g/L;住院时间<7 d 197例(81.1%),平均血清白蛋白浓度为(40.190±4.777)g/L,两组血清白蛋白浓度比较差异具有统计学意义(t=2.697,P<0.05)。结论血清白蛋白浓度检测是婴儿急性腹泻病严重性的有效预测指标之一,在基层医院具有一定临床应用价值。
Objective To investigate the clinical significance of serum albumin concentration in children with acute diarrhea. Methods A total of 243 pediatric acute diarrhea patients were enrolled in this study. The PCIS scores (> 72 points or ≤1) were determined according to whether they were complicated with mild gastroenteritis with benign infantile convulsion (BICE), sepsis or not, 72 points) and different hospitalization days (≥7 d or <7 d). The differences of serum albumin concentration within 24 hours after admission were compared. The mean serum albumin concentration was (40.090 ± 3.504) g / L; 214 cases (88.1%) did not have BICE, mean serum albumin concentration was (39.760 ± 4.871) g / L, there was no significant difference in serum albumin concentration between the two groups (t = 0.352, P> 0.05). (3) In this group of children, PCIS score was less than 72 points in 21 cases (8.6%), mean serum albumin concentration was (36.670 ± 4.643) g / L; PCIS score was 72 points in 222 cases (91.4% The albumin concentration was (40.100 ± 4.632) g / L, the serum albumin concentration was significantly different between the two groups (t = 3.243, P <0.05). (4) Serum albumin concentration (38.130 ± 4.135) g / L in hospitalized time≥7 days (243 cases) was 197 cases (81.1%), average serum albumin The concentration of serum albumin was (40.190 ± 4.777) g / L, the difference was statistically significant (t = 2.697, P <0.05). Conclusion Serum albumin concentration is one of the effective predictors of the severity of acute diarrhea in infants and has certain clinical value in primary hospitals.
