以自行设计的可调式自动活检装置(AABD),在超声导向下对12例血清HBV标记物阳性的慢性乙型肝炎(CHB)病人肝脏进行活检,组织标本进行HBVPCR和Dot-Blot杂交检测并与超声显像参数做相关关系的研究。12例CHB中,5例血清及肝组织HBV-DNA阴性,血清ALT均≤45u/L(HBV-DNA阴性组);7例血清及肝组织HBV-DNA阳性,血清ALT均≥68u/L(HBV-DNA阳性组)。超声检查肝实质回声直方图的像素均值(AV)及其标准差(SD),两组差异显著(P<0.05);慢性肝炎中两组分别与正常对照组比较,超声检查的多项指标的差别具有显著性意义(P<0.05或P<0.01)。多元线性回归与相关分析发现,CHB病人的肝实质回声及其直方图的AV和SD,以及肝内血管走行情况与肝内HBV-DNA的检出存在相关关系(r=0.512,P=0.021,各偏回归系数的P值亦均<0.05),并计算出多元回归方程。
Twelve patients with chronic hepatitis B (CHB) who had serum HBV markers were biopsied with ultrasound-guided biopsy using a self-designed adjustable automatic biopsy device (AABD). The tissues were examined by HBV PCR and Dot-Blot hybridization Ultrasound imaging parameters to do the correlation between the research. Among the 12 CHB cases, HBV-DNA was negative in both serum and liver tissues, serum ALT levels were all less than 45u / L (HBV-DNA negative group). Serum and liver HBV-DNA were positive in 7 cases and serum ALT was 68u / L HBV-DNA positive group). Echocardiography was used to examine the pixel mean (AV) and standard deviation (SD) of the liver parenchyma histogram, with significant differences between the two groups (P <0.05). In the chronic hepatitis group, compared with the normal control group, The difference of indexes has significant significance (P <0.05 or P <0.01). Multivariate linear regression and correlation analysis showed that the AV and SD of the liver parenchymal echo and histogram of CHB patients and the intrahepatic blood vessels were correlated with the detection of intrahepatic HBV-DNA (r = 0.512, P = 0.021, P values of partial regression coefficients were all <0.05), and multiple regression equation was calculated.
