目的：探讨和分析尿中性粒细胞明胶酶相关脂质运载蛋白(uNGAL)在重型颅脑外伤患者并发急性肾功能损伤(AKI)早期诊断价值。方法对我院收治的252例脑外伤患者的uNGAL进行监测，同时对AKI的发生情况进行统计。结果252例患者中，发生AKI患者36例，没有发生的患者为216例。AKI患者的uNGAL值为(7.50±2.64)mg/L要明显比较没发生AKI患者(1.89±1.27) mg/L高，差异显著，(t=35.785，P<0.05)。 uNGAL对脑外伤患者发生 AKI具有明显性的预测价值，随着uNGAL值的升高，患者脑外伤后AKI的敏感性就会下降，但是其的特异性会明显性的升高。结论在临床上，对uNGAL进行监测，能够对脑外伤患者发生 KAI具有很好的早期诊断价值。
