目的:探讨高频振荡通气(HFOV)对重症新生儿胎粪吸入综合征(MAS)的治疗效果。方法:选取广州医科大学附属第二附属医院近几年MAS患儿,分为HFOV组和常频机械通气(CMV)组。对比(1)两组治疗前后肺氧合功能。(2)两组机械通气时间和氧疗时间。(3)两组病情转归及并发症。结果:HFOV组在上机后2 h、12 h、24 h、48 h氧浓度,氧合指数明显低于CMV组,差异具有统计学意义(P<0.05)。HFOV组上机时间,氧疗时间短于CMV组(P<0.05)。结论:HFOV能更好地改善MAS患儿的肺氧合功能,缩短上机时间,对治疗重症新生儿MAS具有较好的疗效。
Objective: To investigate the therapeutic effect of high frequency oscillatory ventilation (HFOV) on severe neonatal meconium aspiration syndrome (MAS). Methods: The second affiliated hospital of Guangzhou Medical University in recent years MAS children were divided into HFOV group and the frequency of mechanical ventilation (CMV) group. Comparison (1) before and after treatment of pulmonary oxygen function. (2) two groups of mechanical ventilation time and oxygen therapy time. (3) two sets of disease outcome and complications. Results: The oxygen concentration and oxygenation index in HFOV group were significantly lower than those in CMV group at 2 h, 12 h, 24 h and 48 h after operation. The difference was statistically significant (P <0.05). HFOV group on-machine time, oxygen therapy time shorter than the CMV group (P <0.05). Conclusion: HFOV can better improve pulmonary oxygenation in children with MAS, shorten the time on the machine, and has a good curative effect on the treatment of severe neonatal MAS.
