目的探讨基因芯片技术检测宫颈癌及癌前病变石蜡标本中,人乳头瘤病毒(HPV)感染的临床意义。方法回顾性分析153份石蜡组织标本,提取组织中DNA,应用基因芯片检测23种HPV亚型,即PCR扩增后产物在基因芯片上杂交。结果在宫颈炎组织中,基因芯片检测出的高危型HPV感染率为8.33%,CINⅠ45.83%,CINⅡ/Ⅲ87.50%,宫颈癌92.21%,其中鳞癌高危型HPV感染率为94.12%,腺癌为88.46%;HPV16为主要的高危致病亚型,占88.98%;HPV18为第2位高危致病亚型,占33.06%;HPV52、33、59、68为少数感染的高危亚型;宫颈鳞癌的HPV16感染率为93.73%,HPV18感染率为27.08%;宫颈腺癌的HPV16感染率为82.61%,HPV18为52.17%;而低危型HPV在宫颈炎中的感染率为8.33%,CINⅠ为16.67%,主要为HPV6、11亚型,CINⅡ/Ⅲ及宫颈癌组织未检测出低危型HPV感染;宫颈炎中HPV均为单一感染,CINⅠ中HPV混合感染率为20.00%,CINⅡ/Ⅲ为28.57%,宫颈癌为36.62%。结论基因芯片技术可应用于石蜡组织标本,检测多种HPV亚型,特异性强,敏感性高,对宫颈癌及癌前病变的防治等研究均具有重要意义。
Objective To investigate the clinical significance of detecting human papillomavirus (HPV) infection in cervical cancer and precancerous lesions by using gene chip technique. Methods A retrospective analysis of 153 samples of paraffin tissue, tissue DNA was extracted, the application of gene chip detection of 23 HPV subtypes that PCR products amplified in the gene chip hybridization. Results In cervicitis tissues, the infection rate of high-risk HPV was 8.33%, CINⅠ45.83%, CINⅡ / Ⅲ87.50%, and cervical cancer 92.21%. The infection rate of high-risk HPV in squamous cell carcinoma was 94.12% Adenocarcinoma was 88.46%; HPV16 was the main high risk causative subtype, accounting for 88.98%; HPV18 was the second subtype of high risk causative disease, accounting for 33.06%; HPV52, 33, 59 and 68 were the high risk subtypes of minority infection; Cervical squamous cell carcinoma of the HPV16 infection rate was 93.73%, HPV18 infection rate was 27.08%; Cervical adenocarcinoma HPV16 infection rate was 82.61%, HPV18 was 52.17%; and low-risk HPV in cervicitis infection rate was 8.33% CINⅠ was 16.67%, mainly HPV6,11 subtype, CINⅡ / Ⅲ and cervical cancer tissues were not detected low-risk HPV infection; HPV were single infection in cervicitis, CINⅠ HPV infection rate was 20.00%, CINⅡ / Ⅲ was 28.57% and cervical cancer was 36.62%. Conclusion Gene chip technology can be applied to paraffin tissue samples to detect a variety of HPV subtypes, strong specificity, high sensitivity, the prevention and treatment of cervical cancer and precancerous lesions are of great significance.
