目的:研究妊娠期肝内胆汁淤积症患者外周血清抑制素A水平及其与孕酮和雌二醇的相关性。方法:选择2014年1月~2015年9月在我院进行诊治的妊娠期肝内胆汁淤积症患者100例,根据病情程度分为轻度组和重度组,另选择20例健康孕妇为对照组。观察并比较三组血清抑制素A、孕酮和雌二醇水平,及血清抑制素A水平与孕酮和雌二醇的相关性。结果:与对照组相比,轻度组和重度组的血清抑制素A均明显升高(P<0.05),且重度组的升高幅度明显大于轻度组(P<0.05);与对照组相比,轻度组和重度组的孕酮水平阳性率和雌二醇水平阳性率明显高于正常对照组(P<0.05),且重度组明显高于轻度组(P<0.05);对照组血清抑制素A水平与孕酮(r=0.15,P>0.05)和雌二醇(r=0.21,P>0.05)水平不具有明显相关性,轻度组血清抑制素A水平与孕酮(r=0.36,P<0.05)和雌二醇(r=0.38,P<0.05)水平均呈明显正相关,重度组血清抑制素A水平与孕酮(r=0.57,P<0.05)和雌二醇(r=0.59,P<0.05)水平均呈明显正相关。结论:血清抑制素A、孕酮和雌二醇可能参与了妊娠肝内胆汁淤积症的发生、发展过程,联合检测血清抑制素A、孕酮和雌二醇有可能成为妊娠肝内胆汁淤积症诊断、分级的新分子标记物。
Objective: To study the serum level of inhibin A in patients with intrahepatic cholestasis of pregnancy and its correlation with progesterone and estradiol. Methods: 100 patients with intrahepatic cholestasis of pregnancy who were diagnosed and treated in our hospital from January 2014 to September 2015 were divided into mild group and severe group according to the severity of disease. Another 20 healthy pregnant women were selected as control group . The levels of serum inhibin A, progesterone and estradiol and the correlation between serum inhibin A level and progesterone and estradiol were observed and compared. Results: Compared with the control group, the serum inhibin A levels in mild and severe groups were significantly increased (P <0.05), and the increase in severe group was significantly greater than that in mild group (P <0.05) The positive rate of progesterone and the positive rate of estradiol in mild group and severe group were significantly higher than those in normal control group (P <0.05), and those in severe group were significantly higher than those in mild group (P <0.05) The level of serum inhibin A was not significantly correlated with the levels of progesterone (r = 0.15, P> 0.05) and estradiol (r = 0.21, P> 0.05) (r = 0.36, P <0.05) and estradiol (r = 0.38, P <0.05) .Serum inhibin A levels were significantly correlated with those of progesterone (r = 0.57, Alcohol (r = 0.59, P <0.05) showed a significant positive correlation. Conclusions: Serum inhibin A, progesterone and estradiol may be involved in the occurrence and development of intrahepatic cholestasis of pregnancy. Combined detection of serum inhibin A, progesterone and estradiol may become intrahepatic cholestasis of pregnancy Diagnosis, classification of new molecular markers.
