目的研究胃癌的螺旋CT征象与组织分化之间的关系。方法55例进展期胃癌病人术前行低张力水充盈螺旋CT三期增强扫描,并将螺旋CT结果与术后病理及组织分化结果进行对照。结果(1)螺旋CT上病灶大小、强化特点与胃癌组织分化密切相关(P<0.05);(2)邻近脏器侵犯者均为低分化癌(P<0.05);(3)高/中分化癌61.1%无淋巴结转移,仅16.7%N2转移;低分化癌86.5%发生淋巴结转移,其中N2转移占71.9%(P<0.05);(4)高/中分化癌66.7%为Ⅰ、Ⅱ期,低分化癌89.2%为Ⅲ、Ⅳ期(P<0.05)。结论螺旋CT上病灶的浸润转移与胃癌分化程度密切相关。
Objective To study the relationship between spiral CT features and tissue differentiation in gastric cancer. Methods Fifty-five patients with advanced gastric cancer underwent three-stage contrast-enhanced spiral CT scanning before operation, and the results of spiral CT were compared with the results of postoperative pathological and histological differentiation. Results (1) The size and enhancement of lesions on spiral CT were closely related to the differentiation of gastric cancer (P<0.05); (2) The adjacent organs were poorly differentiated (P<0.05); (3) High/medium differentiation No lymph node metastasis was found in 61.1% of cancers and only 16.7% in N2 metastasis. Lymph node metastasis was found in 86.5% of poorly differentiated carcinomas. N2 metastasis accounted for 71.9% (P<0.05); (4) 66.7% of high/medium differentiation cancers were stage I and II. 89.2% of poorly differentiated cancers were in stage III and IV (P<0.05). Conclusion The invasion and metastasis of lesions on spiral CT are closely related to the differentiation of gastric cancer.
