目的:探讨温盐水腹腔灌洗对大鼠急性坏死性胰腺炎(ANP)的影响及机制。方法:75只SD大鼠随机分均为假手术组、ANP模型组(模型组)、温盐水腹腔灌洗预处理+ANP模型组(预处理组),术后6 h处死大鼠,检测各组大鼠血清血清淀粉酶与细胞因子含量,PCR法检测胰腺组织HSP70、p38MAPK的mRNA表达,免疫组化法检测胰腺中NF-κB的表达。结果:与假手术组比较,模型组与预处理组大鼠血清淀粉酶、TNF-α、IL-6、IL-8水平均明显升高,但预处理组各指标的升高程度明显低于模型组,模型组与预处理组胰腺组织HSP70、p38MAPK的mRNA表达均明显升高,但预处理组HSP70 mRNA表达量高于模型组,p38MAPK mRNA表达量低于模型组,差异均有统计学意义(均P<0.05)。假手术组大鼠胰腺组织NF-κB呈弱阳性表达,模型组呈强阳性表达,而预处理组呈中等阳性表达。结论:温盐水腹腔灌洗可能通过诱导HSP70表达而抑制p38MAPK及NF-κB活性、下调细胞因子的表达,进而改善大鼠ANP病理生理过程。
Objective: To investigate the effect and mechanism of warm saline peritoneal lavage on acute necrotizing pancreatitis (ANP) in rats. Methods: Seventy-five SD rats were randomly divided into sham-operation group, ANP model group (model group), warm salt water peritoneal lavage pretreatment + ANP model group (pretreatment group) The levels of serum amylase and cytokines in serum of rats were determined. The mRNA expression of HSP70 and p38MAPK in pancreas were detected by PCR and the expression of NF-κB in pancreas was detected by immunohistochemistry. Results: The levels of serum amylase, TNF-α, IL-6 and IL-8 in model group and pretreatment group were significantly higher than those in sham operation group The mRNA expression of HSP70 and p38MAPK in pancreas of model group, model group and pretreatment group were significantly increased, but the expression of HSP70 mRNA in pretreatment group was higher than that in model group, the expression of p38MAPK mRNA was lower than that in model group (All P <0.05). The expression of NF-κB in pancreatic tissue of rats in sham-operation group was weakly positive while the expression of NF-κB in the model group was weakly positive. Conclusion: Intraperitoneal lavage of warm saline can inhibit the expression of HSP70 and p38MAPK, decrease the expression of cytokines, and then improve the pathophysiological process of ANP in rats.
