[目的]探讨围手术期快优康复护理在老年股骨颈骨折微创全髋关节置换术围手术期应用的意义。[方法]2011年1月~2015年12月,本院收治股骨颈骨折患者93例,男33例,女60例;年龄55~90岁,平均(73±5.6)岁,在硬膜外麻醉或全麻下,行后外侧小切口全髋关节置换术治疗,围手术期采用快优康复护理程序,包括术前宣教、术前功能训炼及术后康复程序。[结果]所有患者在术后6周内无皮肤坏死、深静脉血栓、切口感染并发症。术后初次扶拐下床活动时间为2.8 d,末次随访Harris评分(85±7.6)分,较术前(48±11.7)分明显改善。[结论]后外侧小切口全髋关节置换术创伤小,术后疼痛轻,围手术期快优康复护理大大加快了患者康复进程,减少了术后并发症,达到了功能康复。
[Objective] To explore the significance of perioperative fast recovery rehabilitation in the perioperative application of minimally invasive total hip arthroplasty in elderly femoral neck fractures. [Methods] From January 2011 to December 2015, 93 patients with femoral neck fracture were treated in our hospital. There were 33 males and 60 females, aged 55-90 years (average, 73 ± 5.6 years) with epidural anesthesia Or general anesthesia, posterior lateral small incision total hip arthroplasty, perioperative fast recuperation nursing program, including preoperative preaching, preoperative functional training and postoperative rehabilitation procedures. [Results] All patients had no skin necrosis, deep venous thrombosis and incision infection complications within 6 weeks after operation. The first time after bedridden operation was 2.8 d, and the Harris score at the last follow-up was (85 ± 7.6) points, which was significantly improved compared with 48 ± 11.7 (preoperative). [Conclusion] Total hip replacement with posterior lateral small incision is less traumatic and postoperative pain is lighter. Perioperative fast recovery rehabilitation greatly speeds up the rehabilitation process, reduces postoperative complications and achieves functional rehabilitation.
