目的 :回顾Nd :YAG激光在应用中所发生的并发症 ,系统分析了出现的原因 ,提出了治疗方法及防治对策。方法 :复习 1995年 5月至 2 0 0 0年 5月计 1871例 2 480只眼激光爆破中出现的并发症 :包括炎性反应、一过性高眼压、角膜内皮点状混浊、前房出血、人工晶状体损伤等 ,并对相关的指标进行了量化处理。结果 :青光眼虹膜穿孔术 :炎性反应 94 4% ,一过性高眼压 70 6 % ,前房出血 2 8 2 % ,角膜内皮点状混浊 15 4% ,虹膜后粘连 0 5 5 % ,虹膜根部离断 0 10 % ,视网膜出血0 0 5 % ;人工晶状体前膜切开术 :炎性反应 2 5 % ,一过性高眼压 37 5 % ,前房出血 40 6 % ,人工晶状体损伤 2 1 9% ;人工晶状体后囊膜切开 :炎性反应 2 0 1% ,一过性高眼压 18 4% ,前房出血 0 72 % ,人工晶状体损伤 47 6 %。结论 :Nd :YAG激光在眼前节中的治疗作用是安全确实的 ,其并发症的出现与机器的性能、操作者的经验、疾病的种类及程度、患者的配合等密切相关 ,了解这些因素有针对性的干预可避免或减少并发症的发生
OBJECTIVE: To review the complications occurred in the application of Nd: YAG laser, systematically analyze the causes and propose the treatment methods and prevention and cure measures. Methods: From May 1995 to May 2000, a total of 1871 eyes were retrospectively reviewed. The complications of laser blasting were retrospectively analyzed. They included inflammatory reaction, transient ocular hypertension, corneal endothelial opacity, anterior chamber Bleeding, intraocular lens damage, etc., and related indicators were quantified. Results: Glaucoma iris perforation was 94 4%, transient high intraocular pressure 70 6%, an anterior chamber hemorrhage 28 2%, corneal end point opacity 154%, posterior iris adhesion 0.55%, iris 10 0% of the root disconnection, retinal hemorrhage 0 0 5%; anterior lens incision: inflammatory response 25%, transient high intraocular pressure 37 5%, an anterior chamber hemorrhage 40 6%, intraocular lens damage 2 I 9%; intraocular lens incision: 20% inflammatory response, transient intraocular pressure 18 4%, an anterior chamber hemorrhage 72%, intraocular lens injury 47 6%. Conclusion: The therapeutic effect of Nd: YAG laser in the anterior segment is safe and accurate. The occurrence of complications is closely related to the performance of the machine, the operator’s experience, the type and degree of the disease, the cooperation of the patients and so on. Targeted intervention can prevent or reduce the occurrence of complications
