目的:分析耳后注射联合口服激素治疗平坦下降型突发性聋的临床疗效。方法:收集2015年以来收治的128例平坦下降型突发性聋患者的资料,随机分为耳后注射组(64例,注射用甲泼尼龙琥珀酸钠40mg加利多卡因2.5ml,耳后沟局部注射,隔日1次,联合晨起顿服甲泼尼龙片24mg,连服3d)和非耳后注射组(64例,晨起顿服甲泼尼龙片24mg,连服3d)。采用SPSS 17.0软件对2组患者的听力疗效进行统计分析。结果:(1)耳后注射组中15例(23.4%)治愈,9例(14.1%)显效,24例(37.5%)有效,16例(25.0%)无效,总有效率为75.0%(48/64);非耳后注射组中13例(20.3%)治愈,7例(10.9%)显效,21例(32.8%)有效,23例(35.9%)无效,总有效率为64.1%(41/64);2组患者疗效对比差异有统计学意义(P<0.05);(2)耳后注射组250~8 000 Hz中的低、中、高频等5个不同频率段提高依次为(26.48±6.23)dBHL,(23.24±4.21)dBHL,(21.89±7.34)dBHL,(26.07±7.01)dBHL,(25.75±5.14)dBHL。耳后注射组250~500 Hz及250~4 000 Hz频率段听力改善明显高于非耳后注射组,差异有统计学意义(P<0.05)。结论:耳后注射联合口服激素治疗平坦下降型突发性聋患者疗效较口服激素更佳。
Objective: To analyze the clinical effect of ototoxicity combined with oral hormones in the treatment of sudden decline of flat deafness. METHODS: We collected data from 128 patients with sudden decline in sudden deafness who were treated since 2015 and were randomly assigned to receive an auricular injection (64, methylprednisolone sodium succinate 40mg plus lidocaine 2.5ml, Ditch local injection, every other day, combined with morning meal Dayton methylprednisolone tablets 24mg, and even served 3d) and non-auricular injection group (64 cases, Dayton served methylprednisolone tablets 24mg, with service 3d). The SPSS 17.0 software was used to analyze the hearing effects of two groups of patients. Results: (1) 15 cases (23.4%) were cured in the postembryonic injection group, 9 cases (14.1%) were effective, 24 cases (37.5%) were effective, 16 (25.0%) were ineffective and the total effective rate was 75.0% / 64). In the non-auricular group, 13 cases (20.3%) were cured, 7 cases (10.9%) were effective, 21 cases (32.8%) were effective, 23 cases (35.9%) were ineffective and the total effective rate was 64.1% / 64). There was significant difference in the curative effect between the two groups (P <0.05). (2) The five different frequency bands of low, medium and high frequency in 250-8000 Hz after ear injection were ( 26.48 ± 6.23) dBHL, (23.24 ± 4.21) dBHL, (21.89 ± 7.34) dBHL, (26.07 ± 7.01) dBHL, and (25.75 ± 5.14) dBHL respectively. The hearing improvement in 250 ~ 500 Hz and 250 ~ 4 000 Hz in the postemporal injection group was significantly higher than that in the non-auricular injection group (P <0.05). Conclusion: The effect of oral administration of oral hormones in patients with sudden decline of flat deafness is better than that of oral hormone.
