湘谭电缆厂一台SHL20-25锅炉在1992年检修及酸洗除垢后,于5月3日送汽,在5月6日发现炉膛冒白汽,随之发现水位表水位下降为零,“叫水”不上,紧急停炉。经检查,发现炉膛后拱第一排左数第11根凝渣管胀粗,胀粗管长达5.7m,在靠近下部弯头处向火侧,发生鼓包并爆破,鼓包处最大直径为87.9mm(管子为ρ70×3),爆管裂口长55mm,宽10mm,在弯头鼓包侧,有水垢凝渣堆塞。
