目的探讨三维计算机断层扫描血管造影(3D-CTA)对颅内动脉瘤的临床诊断价值。方法经普通CT诊断为蛛网膜下腔出血而高度怀疑颅内动脉瘤的患者38例,均行3D-CTA和DSA检查并同时获得CT三维图像;对颅内动脉显示情况、动脉瘤检出情况、动脉瘤与载瘤动脉及颅骨解剖关系进行回顾性分析。结果与DSA一样,CTA图像可清晰显示颅内动脉主干及其分支。动脉瘤确诊患者35例,检出动脉瘤42个;其中,DSA检出动脉瘤40个,CTA检出动脉瘤41个。CTA对25个瘤颈形态及与载瘤血管三维立体结构清晰显示,DSA仅对8个动脉瘤颈清晰显示(P<0.05)。结论 3D-CTA可以作为临床筛选诊断颅内动脉瘤的常规影像学检查方法,能够为临床治疗方案的选择提供参考依据。
Objective To investigate the value of three-dimensional computed tomography angiography (3D-CTA) in the diagnosis of intracranial aneurysms. Methods Thirty-eight patients with highly suspected intracranial aneurysm who were diagnosed as subarachnoid hemorrhage by general CT were examined with 3D-CTA and DSA and three-dimensional CT images were obtained simultaneously. The status of intracranial artery and aneurysm detected , Aneurysm and the parent artery and the anatomy of the skull for retrospective analysis. Results Like DSA, CTA images clearly show the intracranial arteries and their branches. Thirty-five aneurysms were diagnosed and 42 aneurysms were detected. Among them, 40 aneurysms were detected by DSA and 41 aneurysms were detected by CTA. CTA clearly showed 25 neck neoplasms and three-dimensional structure with host vessels, DSA showed only 8 neck aneurysms clearly (P <0.05). Conclusion 3D-CTA can be used as a routine imaging method for clinical diagnosis of intracranial aneurysms, which can provide reference for clinical treatment options.
