目的探讨3D-TOF序列和3DFIESTA序列联合应用对面肌抽搐及三叉神经痛患者血管压迫病因的诊断价值。方法回顾性分析了14例面肌抽搐及三叉神经痛患者磁共振表现。结果14例患者症状侧有神经血管压迫、接触或可疑接触者共有13侧,1侧为听神经瘤压迫。无症状侧只有2侧可疑神经血管接触,12侧无血管接触征象。统计学结果面肌抽搐及三叉神经痛患者症状侧与无症状侧比较有显著性差异(P<0.001)。结论3DTOFMRA和3DFIESTA成像结合不仅可以显示颅内段颅神经血管压迫情况,并可显示局部肿瘤性病变引起的继发性疼痛原因,为临床医师提供术前评估和指导治疗有极其重要的意义。
Objective To investigate the diagnostic value of 3D-TOF sequence and 3DFIESTA sequence in the diagnosis of vascular compression in patients with hemifacial spasm and trigeminal neuralgia. Methods The MRI findings of 14 patients with facial muscle twitch and trigeminal neuralgia were retrospectively analyzed. Results Fourteen patients had neurovascular compression on the symptomatic side. There were 13 sides of the contact or suspicious contacts, while the other side had oppression of acoustic neuroma. Asymptomatic side only 2 suspects neurovascular contact, 12 sides without signs of vascular contact. Statistical results Facial muscle twitching and trigeminal neuralgia in patients with symptoms and asymptomatic side were significantly different (P <0.001). Conclusion 3DTOFMRA and 3DFIESTA imaging can not only show intracranial cranial neurovascular compression, but also show the causes of secondary pain caused by local neoplastic lesions. It is of great significance for clinicians to provide preoperative evaluation and guidance.
