目的了解Haller气房解剖特点和临床意义。方法对252例疑似慢性鼻窦炎的患者行鼻窦冠状CT检查,了解Haller气房的解剖特点;对鼻窦炎患者行鼻内镜手术,术后随访以了解Haller气房与疗效的关系。结果252例患者中有89例出现Haller气房,总发生率为35%,其中单侧58例,发生率为23%,双侧31例,发生率为12%,双侧发生率较低,而且有明显不对称性,包括大小和左右发生率;252例中慢性鼻窦炎168例,存在Haller气房53例,比较非慢性鼻窦炎组与慢性鼻窦炎组,差异无统计学意义(P>0.05),提示鼻窦炎发生与Haller气房无必然联系;术后随访发现Haller气房残存有13例,无效8例,无Haller气房残存的40例中无效6例,两者比较差异有统计学意义(P<0.01),提示鼻内镜手术疗效与Haller气房有关,是复发原因之一。结论Haller气房具有个体差异及不对称性的特点,不是鼻窦炎的必然病因,熟悉Haller气房的解剖特点可能是提高鼻内镜手术疗效的关键因素之一。
Objective To understand the anatomic features and clinical significance of Haller air cells. Methods 252 patients with suspected chronic sinusitis underwent coronal CT examination to understand the anatomical features of Haller’s air space. Sinusitis patients underwent endoscopic sinus surgery and postoperative follow-up to understand the relationship between Haller’s atrium and therapeutic effect. Results Among the 252 patients, 89 cases appeared Haller air space, with a total incidence of 35%. Among them, 58 cases were unilateral, the incidence was 23%, 31 cases were bilateral, the incidence was 12%, the incidence of bilateral was low, There are obvious asymmetry, including the size and the incidence of left and right; 252 cases of chronic sinusitis in 168 cases, Haller air chamber in 53 cases, compared with non-chronic sinusitis group and chronic sinusitis group, the difference was not statistically significant (P> 0.05), suggesting that there is no relationship between the occurrence of sinusitis and the Haller air space; postoperative follow-up found Haller air space remnants in 13 cases, 8 cases, Haller air room remnants of 40 cases, 6 cases were ineffective, the difference between the two statistics Significance (P <0.01), suggesting that the efficacy of endoscopic sinus surgery and Haller atrium, is one of the causes of recurrence. Conclusion The Haller chamber has the characteristics of individual difference and asymmetry, which is not the inevitable cause of sinusitis. Familiarity with the anatomical features of Haller chamber may be one of the key factors to improve the efficacy of endoscopic sinus surgery.
