基底动脉闭塞是严重的急性神经系统疾病之一,几乎无法使其恢复正常的功能。许多病人入院后病情继续恶化,需要气管插管以维持呼吸通畅,另一些病人呼吸异常,导致气体交换不良。以往的报道没有强调机械性通气对判断这些病人预后的价值。作者复习了一组急性基底动脉闭塞安置机械性通气病人的资料,试图探讨如下问题:(1)呼吸异常能否判断预后?(2)哪种病人能成功地脱离呼吸机?(3)如需要机械性通气,哪种病人不可能从积极的治疗措施中得益?
Basilar artery occlusion is one of the most serious acute neurological diseases that almost fails to return to normal function. Many patients continue to deteriorate after admission, requiring endotracheal intubation to maintain smooth breathing and other patients with abnormal breathing, leading to poor gas exchange. Previous reports did not emphasize the value of mechanical ventilation in judging the prognosis of these patients. The authors reviewed the data of a group of patients with mechanical ventilation of patients with acute basilar artery occlusion and sought to explore the following questions: (1) Can the abnormality of the breath determine the prognosis? (2) Which patient can successfully escape from the ventilator? (3) Mechanical ventilation, which patients can not benefit from aggressive treatment?
