癫痫的外科治疗最重要的前题在于病灶部位的正确诊断。过去应用详细分析癫痫发作内容和头皮EEG的方法是难以确定准确的病灶的立体部位。随着PET(正电子CT)的应用,能够在机体内三维地观察脑代谢情况,又不增加任何损害。由于应用PET了解到癫痫病灶在发作间期处于脑代谢低下状态,并与周围组织区分明显。许多研究者都证实了这一发现。目前PET似乎是最有能力诊断癫痫病灶部位的方法之一。截至目前作者应用PET研究了73例癫痫病人,单纯部分发作32例,复杂部分发作32例,全身发作6例、其他3例。采用~(11)C-glucose口服法测定脑葡萄糖代谢(CMRG);采用C~(15)O_2和~(15)O_2持续吸入法测定脑局部血流量(rCBF)、脑氧消耗量(CMRO_2)。~(11)C和~(15)O都是由JSW小型回旋加速器产生的。不管
