目的了解上海市解脲脲原体(ureaplasma urealyticum,UU)和人型支原体(mycoplasma hominis,MH)感染及其耐药情况,为有效防治支原体感染提供可靠的依据。方法收集2011年1月—2015年12月本院泌尿生殖道标本9 074份,使用支原体培养鉴定计数药敏试剂盒对其进行检测及药敏试验。结果支原体阳性3 238份(占35.68%),其中UU阳性2 505份(占27.61%)。女性宫颈分泌物、男性尿道分泌物及男性精液支原体阳性率依次为53.19%、33.55%、13.61%。男性和女性分别在26~30岁和16~20岁年龄段支原体阳性率最高。除交沙霉素外,多数大环内酯类药物对UU+MH阳性和MH阳性的耐药率均较高。四环素类药物对UU+MH阳性、UU阳性和MH阳性三者的敏感率均较高(>95%)。结论上海地区泌尿生殖道支原体感染以UU为主,在性活跃的人群中高发,且与男性不育有关联。在临床治疗中,可优先考虑采用四环素类药物。
Objective To understand the infection and drug resistance of ureaplasma urealyticum (UU) and mycoplasma hominis (MH) in Shanghai and provide a reliable basis for effective prevention and treatment of mycoplasma infection. Methods Nine 074 samples of urogenital tract from January 2011 to December 2015 in our hospital were collected and tested using Mycoplasma cultivation identification counting susceptibility kit. Results Mycoplasma positive 3 238 (35.68%), of which 2 505 UU positive (27.61%). Female cervical secretions, male urethral secretions and male serovars Mycoplasma positive rates were 53.19%, 33.55%, 13.61%. Mycoplasma positive rate was the highest in males and females at 26-30 years old and 16-20 years old respectively. In addition to Josamycin, the majority of macrolide drugs on the UU + MH positive and MH positive resistance rates were higher. Tetracyclines have a high sensitivity (> 95%) to both UU + MH positive, UU positive and MH positive. Conclusion Mycoplasma genital tract infections in Shanghai are predominantly UU, high incidence in the sexually active population, and associated with male infertility. In clinical treatment, priority may be given to the use of tetracycline drugs.
