目的:观察降脂清肠汤联合奥曲肽治疗高脂血症性急性胰腺炎的临床疗效。方法:选取我院确诊为高脂血症性急性胰腺炎(HL-AP)患者63例,按随机原则分为实验组(n=32)与对照组(n=31),对照组在常规治疗基础上给予奥曲肽治疗,实验组在对照组基础上给予降脂清肠汤胃管内注入,2周为1个疗程。观察两组治疗前后症状指标、实验室指标以及评分指标等变化。结果:治疗2周后,实验组,VAS评分显著优于对照组(P<0.01);两组并发症之间无显著差异性(P>0.05)。治疗2周后,实验组,全血黏度高切、全血黏度低切和血浆粘度三项指标均优于对照组(P<0.05);实验组TC、TG、LDL-C三项指标也优于对照组(P<0.05或P<0.01);治疗2周后,对照组和实验组淀粉酶、CRP三项指标比较无差异性(P>0.05)。治疗后两组患Balthazar CT分级,实验组A级患者明显高于对照组(P<0.05)。结果:降脂清肠汤对奥曲肽治疗无显著拮抗作用,且能够有效降低血脂和血黏度,对高脂血症性急性胰腺炎疗效确切,值得推广。
Objective: To observe the clinical efficacy of Jiangzhi Decoction combined with octreotide in the treatment of hyperlipidemic acute pancreatitis. Methods: Sixty-three patients with hyperlipidemic acute pancreatitis (HL-AP) diagnosed in our hospital were randomly divided into experimental group (n = 32) and control group (n = 31) Based on the given octreotide treatment, experimental group on the basis of the control group given lipid-lowering bowel soup into the stomach, 2 weeks for a course of treatment. The changes of symptoms, laboratory indexes and scoring indexes before and after treatment were observed. Results: After 2 weeks of treatment, the VAS scores of the experimental group were significantly better than those of the control group (P <0.01). There was no significant difference between the two groups in the complication (P> 0.05). After 2 weeks of treatment, the experimental group, whole blood viscosity high cut, whole blood viscosity cut and plasma viscosity three indicators were better than the control group (P <0.05); experimental group TC, TG, LDL-C three indicators are also excellent (P <0.05 or P <0.01). After treatment for 2 weeks, there was no difference in amylase and CRP between the control group and the experimental group (P> 0.05). After treatment, Balthazar CT grade was graded in both groups, and Grade A patients in experimental group were significantly higher than those in control group (P <0.05). Results: Jiangzhi Decoction had no significant antagonistic effect on octreotide treatment, and could effectively reduce blood lipid and blood viscosity, and had a definite curative effect on hyperlipidemic acute pancreatitis and worthy of promotion.
