目的探讨食管癌手术中食管胃吻合口漏的成因及减少其发生的方法,充气试验在减少其发生中的意义。方法对168例食管癌切除手术中食管胃一次性钉合后吻合区域由胃管注入一定压力的空气检验吻合口是否漏气。若有漏气,则于该处予以修补。结果术中对发现的5例吻合不良者给予加强缝合。检出率为2.97%。168例中,无一例发生吻合口瘘,吻合不良者经缝合,未发生吻合口瘘。结论术中充气试验能及时发现吻合口缺陷,对预防吻合口瘘的发生起到了良好的预防作用。
Objective To investigate the causes of esophageal and gastric anastomotic leakage in esophageal cancer surgery and to reduce the occurrence of esophageal cancer. The significance of inflation test in reducing its occurrence was discussed. Methods 168 cases of esophageal cancer resection esophagogastric stapling after anastomosis region by the stomach tube into a certain pressure of air to check whether the anastomotic leakage. If there is a leak, repair it there. Results Intraoperative findings of 5 cases of poor anastomosis were given suture. The detection rate was 2.97%. In 168 cases, none of them had anastomotic fistula, but those with poor anastomosis had sutured without anastomotic fistula. Conclusions Intraoperative inflation test can detect anastomotic defects in time and play a good preventive role in preventing anastomotic fistula.
