对本科将近10年间所做全身骨显象2333例中的34例鼻咽癌患者的全身骨象结果进行了分析,并结合复习文献,在鼻咽癌骨转移的发生率、分布特点及与颈部淋巴结转移和颅底骨破坏的关系等方面进行了讨论,其中15例骨显象为异常,结合临床及X线检查确诊为骨转移,其发生率为送检病人的44.1%。骨转移患者全部为多发骨转移,平均每例病灶数8.7个,骨转移的部位以脊柱和胸部为最多,其次为骨盆、肢体、颅骨,肢体转移中以下肢为主(8.9%),上肢少见(2.3%)。结果还表明:鼻咽癌骨转移与患者是否并颅底骨破坏有关(P<0.05),即若患者已诊断有颅底骨的破坏,则骨转移的机率较高。至于鼻咽癌骨转移与患者是否伴有颈部淋巴结转移的关系,虽然本组骨转移患者颈部淋巴结转移的发生率高于非骨转移患者,但未经能统计学处理证实,有待进一步探讨。本文作者认为鼻咽癌转移是通过淋巴遭到达颈部淋巴结群,然后进一步累及颈静脉系统而进入椎体,发生血行转移。其骨转移方式与其它恶性肿瘤发生骨转移的途径相似。
The results of whole body bone imaging of 34 nasopharyngeal carcinoma patients in 2333 cases of whole body bone imaging made in the recent 10 years were analyzed. The authors suggest that NPC metastasis is reached through the lymph nodes of the cervical lymph node group, and then further involved the jugular vein system into the vertebral body, the occurrence of hematogenous metastasis. Its bone metastases are similar to the pathways of bone metastasis in other malignancies.
