目的探讨慢性乙型肝炎患者肝纤维化指标与肝组织纤维化分期及炎症分级的关系,寻找能够较好地反映肝脏组织学损伤的无创检查指标。方法我们对144例慢性乙型肝炎患者进行肝穿刺活检,并采集静脉血检测透明质酸(HA)、层粘连蛋白(LN)、Ⅳ型胶原(CⅣ)和Ⅲ型前胶原N端肽(PⅢNP),分别对这4项指标与肝组织纤维化分期及炎症分级进行相关性分析。结果肝组织纤维化分期及炎症分级与HA、LN、CⅣ和PⅢNP均存在明显正相关(分别为0.627,0.365,0.531,0.435及0.679,0.358,0.582,0.583,值均<0.05)。结论透明质酸、层粘连蛋白、Ⅳ型胶原和Ⅲ型前胶原N端肽对于预测慢性乙型肝炎患者的肝组织损伤有一定的临床意义。“,”Objective To explore the relationship between serum fibrosis indexes and liver fibrosis stages/inflammation grades in patients with chronic hepatitis B, and to find the better noninvasive indexes for liver histological damage. Methods The liver fibrosis stages/inflammation grades in 144 CHB patients were confirmed by pathological examination using biopsy tissues. The serum fibrosis indexes (hyaluronic acid, laminin, collagen typeⅣ, Collagen TypeⅢN-peptide) were tested in the patients. The correlation analysis was made between the aforementioned 4 indexes and liver fibrosis stages/inflammation grades. Results However, significant positive correlations were found between the liver fibrosis stages/inflammation grades and liver fibrosis indexes (Spearman r for HA, LN, CⅣand PⅢNP were 0.627,0.365,0.531,0.435 and 0.679,0.358,0.582,0.583, respectively, all <0.05). Conclusion Hyaluronic acid ,laminin,Collagen typeⅣand Collagen TypeⅢN-peptide may be used as an noninvasive markers for liver injuries in CHB patients.
