丹红注射液联合西药治疗糖尿病下肢动脉硬化闭塞症老年患者疗效分析

目的:观察丹红注射液联合西药治疗糖尿病下肢动脉硬化闭塞症老年患者的临床疗效。方法:将110例糖尿病下肢动脉硬化闭塞症老年患者按随机数字表法分为2组各55例,常规治疗组口服降糖药物或注射胰岛素控制血糖,并严格控制血压、血脂等,同时给予硫辛酸注射液、甲钴胺片、西洛他唑片治疗;中药治疗组在常规治疗基础上配合丹红注射液治疗。2组均连续治疗3月为1疗程,治疗3疗程。分别于治疗前后测定股浅动脉、足背动脉、胫后动脉的血管内径、血流量及双足振动感觉阈值、踝肱指数,评价临床疗效。结果:总有效率中药治疗组为83.6%,常规治疗组为63.6%,2组总有效率比较,差异有统计学意义(P<0.05)。治疗后,中药治疗组股浅、足背、胫后动脉的血管内径及血流量均较治疗前明显改善(P<0.01);常规治疗组足背动脉血管内径及血流量均较治疗前明显改善(P<0.01,P<0.05)。中药治疗组股浅、足背、胫后动脉的血管内径及血流量的改善情况优于常规治疗组(P<0.05)。2组双侧踝肱指数均较治疗前升高(P<0.01),中药治疗组踝肱指数升高情况优于常规治疗组(P<0.05)。结论:丹红注射液联合西药治疗2型糖尿病伴PAD老年患者,可有效增加下肢血管内径与血流量,改善患肢疼痛和间歇性跛行等症状,疗效优于单纯西医常规疗法治疗。 Objective: To observe the clinical effect of Danhong injection combined with western medicine in the treatment of diabetic lower extremity arterial occlusive disease in elderly patients. Methods: A total of 110 cases of diabetic lower extremity arteriosclerosis obliterans elderly patients were divided into two groups of 55 cases by random number table, the conventional treatment group oral hypoglycemic drugs or insulin injection control blood sugar, and strict control of blood pressure, blood lipids, while giving sulfur Caprylic acid injection, mecobalamin tablets, cilostazol tablets treatment; Chinese medicine treatment group on the basis of conventional treatment with Danhong injection treatment. 2 groups were continuous treatment of March for a course of treatment, treatment 3 courses. Before and after treatment, the vessel diameter, blood flow and the bipedal sensory threshold and ankle brachial index of the superficial femoral artery, dorsalis pedis artery and posterior tibial artery were measured respectively to evaluate the clinical curative effect. Results: The total effective rate was 83.6% in the traditional Chinese medicine treatment group and 63.6% in the conventional treatment group. The total effective rate in the two groups was statistically significant (P <0.05). After treatment, the vessel diameter and blood flow in the shallow, dorsal and posterior tibial artery of TCM group were significantly improved compared with that before treatment (P <0.01); the diameter and blood flow of dorsal artery in the conventional treatment group were significantly improved (P <0.01, P <0.05). The improvement of the vascular diameter and blood flow in the superficial, dorsalis pedis and posterior tibial artery of traditional Chinese medicine group was better than that of the conventional treatment group (P <0.05). The ankle-brachial index in both groups was significantly higher than that before treatment (P <0.01). The increase of ankle-brachial index in Chinese medicine group was superior to that of conventional treatment group (P <0.05). Conclusion: Danhong injection combined with Western medicine in the treatment of type 2 diabetes mellitus patients with elderly PAD can effectively increase the diameter and blood flow of the lower extremities, and improve the symptoms of limb pain and intermittent claudication. The curative effect is better than that of conventional western medicine.
