正压通气并发肺损伤表现为肺泡外气体者约占0.5%~25%,但严重急性呼衰或ARDS则可高达88%。一般认为这是气压伤所致,即气道压力过高引起的肺损伤。用动物作实验显示肺过度膨胀的剪切力是损伤的真正原因而不一定是肺泡内压力本身。因为给ARDS瘸人用PEEP的压力高达50cmH_2O气压伤的发生率也不一定增加,临床上显示肺损伤的病人也并非都用了高气道压通气。因此很可能还有另外的机理
Positive pressure ventilation with lung injury manifested as alveolar gas accounted for about 0.5% to 25%, but severe acute respiratory failure or ARDS up to 88%. Is generally believed that this is caused by barotrauma, that is, airway pressure caused by lung damage. Experiments with animals show that the excessive expansion of the lung shear force is the real cause of injury and not necessarily the pressure inside the alveoli itself. Because of the ARDS lame PEEP pressure up to 50cmH_2O pressure injury does not necessarily increase the incidence of clinically show that patients with lung injury are not also used high airway pressure ventilation. So probably there is another mechanism
