本研究旨在运用血流向量成像技术(VFM)研究原发性高血压患者左心室能量损耗差异,尽早检出流场力学异常的高血压患者,早期预防靶器官损害,并初步探索反映原发性高血压患者心脏舒缩功能变化的新方法。纳入健康志愿者22例、左室心肌质量指数(LVMI)正常的高血压患者14例及LVMI升高的高血压患者21例作为研究对象,分别行常规超声心动图、VFM检查及双频脉冲多普勒检查。结果显示高血压病例组的左室能量损耗明显高于正常组(P<0.05),且舒张期能量损耗=0.439×SBP(收缩压)–8.349,收缩期能量损耗=0.385×SBP+0.644×LVMI–10.854;左室舒缩期能量损耗与E/e’相关,但与左室射血分数(EF)不相关。研究表明左室能量损耗有助于发现高血压患者流场力学变化;左室能量损耗变化是否能成为评估左室舒张功能的新指标,尚需深入研究;SBP是舒张期能量损耗的独立预测因素,LVMI及SBP是收缩期能量损耗的独立预测因素。
This study aimed to study the difference of left ventricular energy loss in patients with essential hypertension by using blood flow vector imaging (VFM), to detect hypertensive patients with flow mechanics abnormality as soon as possible, to prevent target organ damage in the early stage, A new method for the change of cardiac systolic and diastolic function in patients with essential hypertension. Twenty-two healthy volunteers, 14 hypertensive patients with normal left ventricular myocardial mass index (LVMI), and 21 hypertensive patients with elevated LVMI were enrolled in this study. Routine echocardiography, VFM, Pule check. The results showed that left ventricular energy loss in hypertension group was significantly higher than that in normal group (P <0.05), and diastolic energy loss was 0.439 × SBP (systolic pressure) -8.349, systolic energy loss was 0.385 × SBP + 0.644 × LVMI -10.854; energy loss in left ventricular systolic and diastolic phase correlated with E / e ’but not with left ventricular ejection fraction (EF). Studies have shown that left ventricular energy loss contributes to the discovery of fluid mechanics changes in hypertensive patients; Whether left ventricular energy loss can be a new index to evaluate left ventricular diastolic function needs further study; SBP is an independent predictor of diastolic energy loss , LVMI and SBP were independent predictors of systolic energy loss.
