目的:观察母体血糖、胰岛素、空腹胎盘催乳素(HPL)水平与胎儿体重之间的关系。方法:对62例妊娠32~36周的正常孕妇进行75g糖耐量试验,同时检测空腹和餐后1h、2h血糖和胰岛素,以及HPL的浓度,并跟踪其胎儿出生体重。结果:胎儿体重与母体餐后血糖及HPL水平均显著正相关,P<001,P<0001;与空腹血糖及胰岛素水平无相关。多重回归分析显示,去除母体糖代谢因素影响后,HPL仍与胎儿体重显著正相关,P<005。结论:生理范围的母体血糖水平变化与胎儿生长有密切关系,HPL对胎儿生长的影响并不完全通过母体糖代谢
Objective: To observe the relationship between maternal blood glucose, insulin, fasting placental lactogen (HPL) and fetal body weight. Methods: A total of 62 pregnant women with gestational age from 32 to 36 weeks were enrolled in the study. The fasting and postprandial blood glucose, insulin, and HPL concentrations were measured at 1 hour and 2 hours after meal, and their birth weight was tracked. Results: There was a significant positive correlation between fetal body weight and postprandial blood glucose and HPL level (P <001, P <0001). No correlation was found between fasting blood glucose and insulin level. Multiple regression analysis showed that, after removing the influence of maternal glucose metabolism, HPL still had a significant positive correlation with fetal body weight, P <005. CONCLUSIONS: Physiological changes in maternal blood glucose levels are closely related to fetal growth, and the effects of HPL on fetal growth are not completely mediated by maternal glucose metabolism
