目的探究药物准备过程中断的次数和持续时间及其导致中断的因素,从而为护理工作人员因药物准备过程中断而造成的医疗失误作出指导。方法连续性观察2015年6月—12月期间护理工作人员的日常工作,记录所有护士期间发生药物准备过程中断的次数和持续时间,并分析导致其中断的因素,对结果进行统计学的处理。结果共收集832个药物准备过程,其中发生中断有751个,平均中断次数为(6.1±0.7)次,平均持续时间为(6.5±2.4)min,导致中断的因素包括医师传唤、患者呼叫、接听电话、家属答疑以及同事交流等人为因素和材料处理、信息录入以及药品发放等非人为因素。结论护理工作人员在进行药物准备的过程中,发生中断的次数频繁,持续的时间长,导致的因素多。因此,护理工作人员应注意分析药物准备过程中断的因素,减少不必要的中断次数,从而降低因药物准备过程中断而造成的医疗失误的发生率。
OBJECTIVE To investigate the number and duration of interruptions in the drug preparation process and the factors that led to the interruptions, so as to provide guidance to care workers on medical errors resulting from interruption of the medication preparation process. Methods Continuity The day-to-day activities of nursing staff during June-December 2015 were observed. The number and duration of interruptions during the preparation of all nurses were recorded and the factors leading to their discontinuation were analyzed and the results were statistically processed. Results A total of 832 drug preparations were collected, including 751 interruptions and 6.1 ± 0.7 interruptions on average, with an average duration of (6.5 ± 2.4) min. The factors that led to disruption included physician summoning, patient call, answering Telephone, family members answering questions and exchange of colleagues and other human factors and material handling, information entry and drug distribution and other non-human factors. Conclusion Nursing staff in the process of drug preparation, the occurrence of frequent interruptions, sustained a long time, leading to many factors. Therefore, caregivers should pay attention to analyzing the factors that interrupt the drug preparation process and reduce the number of unnecessary interruptions, thereby reducing the incidence of medical errors caused by the interruption of the drug preparation process.
