目的应用组织多普勒测量二尖瓣环运动频谱评价重度子痫前期孕妇左心舒张功能。方法测量55例先兆子痫前期患者和80例血压正常孕妇左室射血分数(LVEF)、二尖瓣瓣口舒张早期E波峰值流速与舒张晚期A波峰值流速之比(E/A)及应用组织多普勒测量二尖瓣环处六个点位的舒张期心肌运动频谱Ea、Aa及计算Ea/Aa比值。结果 LVEF与E/A两组比较差异无统计学意义(P>0.05),Ea和Ea/Aa较对照组下降,Aa测值与对照组增高,两组比较差异均有统计学意义(P<0.05)。结论应用组织多普勒测量二尖瓣环处六个点位的舒张期心肌运动频谱可以准确反映重度子痫前期患者左心舒张功能,无假正常化现象。
Objective To evaluate left ventricular diastolic function in patients with severe preeclampsia by measuring mitral annular motion spectrum with tissue Doppler. Methods The left ventricular ejection fraction (LVEF), peak early diastolic E wave velocity and peak A diastolic velocity ratio (E / A) were measured in 55 preeclampsia patients and 80 normotensive pregnant women. Tissue Doppler was used to measure Ea, Aa and Ea / Aa of left ventricular diastolic myocardial motion at six sites at mitral annulus. Results There was no significant difference between LVEF and E / A groups (P> 0.05). The Ea and Ea / Aa levels were lower than those in the control group, and the Aa values were increased in the control group. The differences between the two groups were statistically significant (P < 0.05). Conclusion Tissue Doppler measurement of diastolic myocardial motion spectrum at six points at the mitral annulus can accurately reflect left ventricular diastolic function in patients with severe preeclampsia without false normalization.
