目的:研究人绒毛膜促性腺激素β亚基(β-HCG)在卵巢癌组织中的表达特点,探讨异位β-HCG与卵巢癌组织类型、分化程度之间的关系。方法:选择2006年11月~2011年11月在吉林大学第二医院妇产科住院的64例患者的手术切除标本,其中正常卵巢组织(对照组)10例、卵巢良性肿瘤组织(良性组)15例、卵巢交界性肿瘤组织(交界组)13例、卵巢癌组织(恶性组)26例。根据细胞分化程度不同,恶性组又分为低分化组(13例)、中分化组(7例)及高分化组(6例)。应用免疫组织化学法检测3组β-HCG的表达水平。结果:β-HCG在正常对照组、卵巢良性肿瘤组与卵巢交界性肿瘤组中均无阳性表达,异位β-HCG在卵巢上皮癌中的阳性表达率高达53.8%;高分化组1例阳性,阳性率为16.7%;中分化组3例阳性,阳性率为42.9%;低分化组10例阳性,阳性率76.9%,差异有统计学意义(P<0.05)。结论:异位β-HCG的表达与卵巢恶性肿瘤发生相关,异位β-HCG的阳性表达率与肿瘤分化程度关系密切,分化愈差的癌细胞异位β-HCG的表达率愈高,表达强度愈强,异位β-HCG有望作为新的卵巢肿瘤标志物来帮助判断肿瘤的良恶性及治疗后的病情转归。
OBJECTIVE: To study the expression of human chorionic gonadotropin β subunit (β-HCG) in ovarian cancer and to explore the relationship between the expression of ectopic β-HCG and the histological type and differentiation of ovarian cancer. Methods: Totally 64 patients who were hospitalized in Department of Obstetrics and Gynecology of the Second Hospital of Jilin University from November 2006 to November 2011 were selected for surgical resection. Among them, 10 cases of normal ovarian tissue (control group), 10 cases of benign ovarian tumor (benign group) 15 cases, borderline ovarian tumor (junction group) in 13 cases, ovarian cancer (malignant group) in 26 cases. According to the degree of cell differentiation, the malignant group was divided into poorly differentiated group (13 cases), moderately differentiated group (7 cases) and well differentiated group (6 cases). The expression of β-HCG in three groups was detected by immunohistochemistry. Results: The positive expression rates of β-HCG in normal control group, benign ovarian tumor group and borderline ovarian tumor group were all higher than those in ovarian epithelial carcinoma group , The positive rate was 16.7%; 3 cases in the moderately differentiated group were positive, the positive rate was 42.9%; 10 cases were positive in the poorly differentiated group, the positive rate was 76.9%, the difference was statistically significant (P <0.05). Conclusion: The expression of ectopic β-HCG is correlated with the occurrence of ovarian cancer. The positive rate of ectopic β-HCG is closely related to the degree of tumor differentiation. The higher the expression rate of ectopic β-HCG is, the higher the expression of ectopic β-HCG is The stronger the intensity, ectopic β-HCG is expected as a new ovarian tumor markers to help determine the benign and malignant tumors and prognosis after treatment.
