目的研究星形细胞肿瘤的磁共振波谱(MRS)特点以及不同级别星形细胞肿瘤的MRS改变。资料与方法本研究包括两组:病例组和对照组。均采用GE Signa 3.0 T MR进行检查,MRS采用多体素点分辨波谱(PRESS)序列。病例组52例,男31例,女21例,年龄27~65岁,平均(41±10)岁,均经手术病理证实,其中弥漫性星形细胞瘤20例,间变性星形细胞瘤14例,胶质母细胞瘤18例,体素位置选择在肿瘤实质区。对照组包括26名健康自愿者,其中,男8名,女18名。自愿者的选择要求其平均年龄与病例组相匹配,体素位置选择在双侧半卵圆中心区。结果 (1)星形细胞肿瘤实质区各代谢物均与健康自愿者有显著性差异;(2)星形细胞肿瘤在MRS上常表现为胆碱(Cho)升高、N-乙酰天门冬氨酸(NAA)降低,随着肿瘤恶性程度增加,其Cho/肌酸(Cr)、Cho/NAA升高,并且在高度恶性星形细胞肿瘤中出现脂质(Lip)峰。结论 MRS是一种有潜力的方法,有助于星形细胞肿瘤患者的术前诊断。
Objective To study the characteristics of magnetic resonance spectroscopy (MRS) of astrocytic tumors and the changes of MRS in different grades of astrocytic tumors. Materials and Methods The study included two groups: the case group and the control group. All were examined using the GE Signa 3.0 T MR, which uses a multiple voxel point-resolved spectroscopy (PRESS) sequence. The case group consisted of 52 patients, 31 males and 21 females, aged from 27 to 65 years (mean, 41 ± 10 years), all of whom were confirmed by surgery and pathology. Among them, 20 were diffuse astrocytomas, Cases, glioblastoma in 18 cases, voxel location in the tumor parenchyma area. The control group consisted of 26 healthy volunteers, including 8 males and 18 females. The choice of volunteers requires that their average age match the case group, with the voxel location selected in the bilateral semio-oval center. Results (1) All the metabolites in the astrocytic tumors were significantly different from those in healthy volunteers. (2) Astrocytic tumors often showed increased Cho in MRS, increased N-acetyl aspartate (NAA), with increased Cho / creatinine (Cr) and Cho / NAA (tumor necrosis factor) and increased lipid (Lip) peak in highly malignant astrocytoma. Conclusions MRS is a promising method for the preoperative diagnosis of astrocytoma patients.
