The management of gastric cancer in Japan is a little different from other countries for the following two major reasons.One is the availability of the S-l, and the other is the prevalent use of second line chemotherapy.S-1 is an orally active combination drug consisting of tegafur as a prodrug of 5-FU, gimeracil, and oteracil potassium.Based on the results of domestic phase Ⅲ trials, S-1 is recognized in Japan as one of the key drugs for the treatment of adenocarcinomas, not only gastric but also colorectal, lung and pancreatic, whereas in other countries its introduction has only just about started.JCOG9912 was conducted to investigate the superiority of irinotecan plus CDDP and non-inferiority of S-1 compared with 5-FU alone, and only non-inferiority of S-1 was shown to be statistically significant.
