Rest and stress single photon emission computed tomography (SPECT) remains the most widely applied and most extensively validated noninvasive imaging technique for assessing patients with possible or known coronary artery disease (CAD).Positron emission tomography (PET) represents another nuclear medicine based option for assessing myocardial perfusion that in general provides information similar to that provided by SPECT.There are 3 clinical scenarios where the information provided by PET is superior to that provided by SPECT: (1) Patients with suboptimal SPECT images due to soft tissue attenuation artifacts.Due to high-energy photons and built-in attenuation correction, image quality in general is superior with PET in patients at significant risk of attenuation artifact (obesity, women with large breasts, high diaphragms).
