United States Agency for International Development and Academic Model Providing Access to Healthcare (USAID-AMPATH) partnership, located in Kenya and created to provide HIV management, has expanded to provide diabetes care.Since 2007, more than 2000 patients have enrolled.Treatment decisions are made based on data collected using database collection forms developed by the pharmacy team.Home glucose monitoring is also provided to patients with HbA1c > 12% and access to a cell phone.Patients are given a glucometer and provided with glucose testing strips.They are called weekly by community workers who record the results and relay the information to clinicians who then adjust insulin dosing based on a standardized protocol.An electronic medical record has been developed to capture all aspects of care in this setting.The focus is to advance this model to additional rural sites and to serve as a teaching and research model for resource-constrained settings.
