目的:分析青少年特发性脊柱侧凸(AIS)女性患者瘦素、可溶性瘦素受体(sOB-R)水平与其躯体生长发育特征的关系,探讨瘦素、可溶性瘦素受体浓度在AIS女性患者躯体生长发育特征异常中的可能作用.方法:研究对象包括63例单胸弯AIS女性患者和27例同龄健康女孩.测量所有AIS组和对照组女孩的身高及体重.AIS患者与正常对照青少年的体重指数计算方法为BMI =体重(kg)/臂长2 (m2).运用酶联免疫吸附方法检测AIS组和对照组女孩血清瘦素及可溶性瘦素受体水平.比较生长发育特征相关指标和血清瘦素及可溶性瘦素受体浓度在两组女孩之间的差异.此外,采用多元回归分析影响瘦素、可溶性瘦素受体浓度的独立因素.结果:AIS女孩年龄11-16岁(平均14.1±1.5岁),Cobb角20-88°(平均38.1±18.1°).对照组年龄11-16岁(平均14.2±1.6岁).与同龄对照组女孩相比,AIS女孩有较低的体重、BMI征及较长的臂长.AIS女孩的瘦素浓度低于同龄健康女孩,但无明显差异;而AIS女孩的可溶性瘦素受体水平显著高于正常对照组(p=0.004).多元回归统计结果显示:无论是对于瘦素还是可溶性瘦素受体, BMI均是影响其水平的独立因素;而AIS发生与否是影响可溶性瘦素受体水平的独立因素.结论:胸弯AIS女孩可溶性瘦素受体水平显著高于对照组女孩,同时AIS发病与否是影响可溶性瘦素受体水平的独立因素之一,进一步说明AIS女孩的外周血清中瘦素生物利用度存在明显异常,AIS女性患者的高可溶性瘦素受体水平可能在引起患者低体重和BMI方面起重要作用.
