Objective To evaluate the efficacy of sequential stepwise anti-reflux therapy on refractory gastroesophageal reflux induced cough (GERC).Methods 36 consecutive patients with suspected GERC refractory to the standard pharmacological anti-reflux therapy were recruited and treated with the high dose of proton pump inhibitor (omeprazole 40mg, twice a day) plus prokinetic agent (domperidone 10 mg, three times a day), adding histamine H2 receptor antagonist (ranitidine 20mg, twice a day) to the current treatment and the combination of proton pump inhibitor (omeprazole 20mg, twice a day) with transient lower esophageal sphincter relaxation inhibitor (baclofen 20mg three times daily) in a sequential and step-up way.
