Objective: The free fibula flap is widely used for reconstruction of maxillary defects. However, the three-dimensional position of the fibula flap is very difficult to control during the operation only according to the experience of the surgeons. The purpose of this study is to improve the process by using computer techniques. Methods: 27 patients with maxillary tumor have been performed with maxillectomy and free fibula flap reconstruction in Peking University School of Stomatology from 2011 to 2013.Virtual planning and surgical navigation were used in 8 cases, and 19 cases were operated traditionally by the experience of the surgeons. The three-dimensional position including the vertical distance and horizontal position, as well as the relation between the posterior end of fibula and the coronoid process were evaluated in the two groups. The difference between the postoperative position of the fibula and the virtual planning in computer-assisted group was also evaluated. The data was statistically analyzed with SPSS 17.0.
